Lab Home Visit

Certain lab services are available at home, ONLY if your doctor has requested a home visit, and ONLY if along with Medicare you have Medicare Supplemental Insurance.

Labs in our area include LabCorp and Quest. You must call the lab in your home area. 24 hour advance notice is required. You will need to provide the following information:

• Name, Address, Phone Number
• Medicare & Insurance Company ID Numbers
• Prescription from Doctor with Diagnosis Code
• Which tests are needed
