Lab Home Visit

Certain lab services are available at home.

 

Labs in our area include Gentle Hands and LabCorp. 24-hour advance notice is required. You will need to provide the following information:

 

• Name, address and phone number

• Medicare and insurance company ID numbers

• Prescription from your doctor with diagnosis code

• Which tests are needed
